MEDICAL RELEASE
In the event of an emergency or non-emergency situation in which medical treatment is required for my child(ren) listed above, as a result of participation with the 2013 Vacation Bible School day camp held at Triumph Church, I authorize the on-site adult leaders, volunteers, and staff members to act as my agent and thereby consent to emergency medical treatment for my child.  I understand that every reasonable attempt will be made to contact me in the event of an emergency.

This year, Vacation Bible School is at TWO locations AND it's a full day! And we still kept it FREE for your children!

FREE meals, activities, crafts, volunteer screening/training and activities all cost resources to provide. Your donations will help offset these costs.
